Chalk teeth – the new common disease?

Molar incisor hypomineralization (MIH) is a disease that affects one or more of the first permanent molars (adult molars that arise when the child is 6-7 years old) and occasionally the frontal and second molars. In the Volksmunde the clinical picture is also called chalk teeth designated. Affected teeth have white, yellow or brown spots. First permanent molars with MIH are weaker, more porous, and tend to break in the mouth shortly after they occur. These teeth are more prone to tooth decay and can be sensitive to hot or cold foods and drinks. Restoring these teeth can be challenging, as these teeth are not well stunned, and the filled fillings will come off due to an inability to adhere well to the damaged tooth surface. Front teeth affected by this condition are difficult to restore aesthetically.

What are the causes?

Dentists say that the rot in 90% of cases avoidable where eating habits are the obvious culprit. But far more confusion in modern dentistry is what is behind the decay of those for whom it is unavoidable. Many of these children have what is unrecognized as Molar-incisor hypomineralisation, a condition that was first recognized in the 80s. It means that the outer enamel does not form properly on certain teeth. Some dentists report an increase in the cases they see, but the condition is still poorly understood. If it does, MIH affects the first four adult molars that break through at about the age of six. The condition varies in severity, often only with one tooth affected. But at the very end of the spectrum, a tooth begins to crumble shortly after it breaks through, which is referred to as chalk teeth. Mild cases are only slightly discolored. The enamel is thinner and softer and tends to dissolve. The surface allows bacteria to hide in the defects, which means that these teeth are more prone to rot. Fluoride treatments, along with strict brushing and nutrition regimes, are the only way to save them.

However, as all parents know, one is precise teeth cleaning a challenge in the mouth of a six-year-old child – especially if they have such a condition that makes the affected teeth very sensitive. Dental examinations can be unbearable, so it makes sense to visit a specialist in pediatric dentistry practice.

Even after many academic studies, researchers know little about what causes MIH. The impression is that MIH is a modern disease. But 200 to 300 year old skeletons show the same disease picture. Scandinavian and German researchers found child skeletons born in consecutive years, and instead of increasing steadily, the prevalence is increasing and decreasing. On negative influence of plasticizers in plastic can not be proven by scientific studies.

Prophylaxis and prevention

Occasionally, white or yellowish spots appear on the front teeth of adults, but apart from rare cases, these are only a cosmetic problem that, if it is Well-being of the child impaired, can be masked. When a friable molar has to be removed, not all is lost. If you get the right timing, the second adult molar and wisdom teeth will usually slip forward and take the place.

One of the classic error, The parents commit is that children at night with milk or juice bottles to bed bring. At three in the morning, most parents would do anything to put a child back to sleep, but fruit juice is very damaging to the teeth. Also the permanent rinsing with milk or mineral water damages the teeth at night. Even toddlers carrying juice bottles all day long are keeping their teeth decaying. Moving to cups instead of bottles and limiting the juice to the meal helps to avoid this. Also, snacking should be minimized, with sweet treats being reserved for the pudding rather than between meals. Even things like chips are turned into sugar in the mouth by enzymes. Worth knowing about the spreading of the chalk teeth

Is one MIH disease before, patients have a tremendous sense of pain on their teeth. This can go so far that even brushing your teeth already causes piercing pain and hot and cold drinks are completely avoided. The visual blemishes, on the basis of which the designation "chalk teeth" is therefore by no means the only symptom. Nevertheless, sufferers feel burdened with it as well. The ugly furrows are often recognizable even from a moderate distance, especially when they turn brownish.

According to recent surveys, due to "novelty" the chalk teeth but are hardly long term, the disease occurs in about 10 to 15% of children and adolescents on. In the age group under 12 years will even spread to about 33% documented. softener, a special form of plastics, are repeatedly associated as a potential cause of it. Since MIH can be attributed to a malfunctioning mineralization, a potentially higher susceptibility at young age may supportive.

In science, however, other causes are discussed that are not concretely related to childhood or the plasticizer thesis. For example Changes in hormone balance in pregnancy, but also various infectious diseases, excessive intake of selected antibiotics or a disease with chickenpox.

Treatment of chalk teeth in children and adolescents

A big hurdle of MIH is the treatment. Due to the increased sensation of pain, sometimes even a treatment under partial or general anesthesia is required. How extensive this treatment is depends on the degree of MIH. Anterior teeth, for example, could be visually adjusted with ceramic or plastic fillings, but the fissures are sealed on the surface. This should at the same time put the increased pain sensation.

A severe damage to the tooth, which is the case, among other things, when tooth hard substance is lost, this affects the treatment. One option is then a partial or full crown, which at the same time has a protective effect against the severely damaged tooth. In the worst case, a complete removal of the affected tooth is necessary. The resulting gap could, for example, be closed orthodontically.

With regard to prophylaxis, however, the treatment remains predominantly fluoride and a, as previously mentioned highly concentrated fluoride lacquer mixture.
