The hemorrhoids are blood filled erectile tissue which helps to seal the anal canal.

Depending on their size, hemorrhoids are divided into 4 groups:

1st degree hemorrhoids

  • are normal. They are visible only with the proctoscope and usually do not cause discomfort.

2nd degree hemorrhoids

  • During stool pass through the anal canal to the outside and then pull back on their own. This can often lead to bright red bleeding.

3rd degree hemorrhoids

  • are also pushed outward by the bowel movements, but then have to be pushed back with their fingers because they no longer glide back by themselves. In addition to bleeding, it can also cause oozing, itching and burning in the anus region.

4th degree hemorrhoids

  • are constantly outside the anal canal, often combined with an after-injury. They can not be pushed back into the anal canal with their fingers. You feel them as soft bulging knots.

Depending on how much the hemorrhoids are enlarged, then put on the complaints, such as itching and burning in the anus region, oozing or relubrication (soiled underwear). Extremely enlarged hemorrhoids can be felt as a knot, here it also comes to a pressure or foreign body sensation in the anus region and to the feeling of incomplete evacuation.

One of the first symptoms is the anal bleeding. It can be light (a few blood spatters on the toilet paper) or so strong that blood drips into the toilet bowl. Since one never knows where the bleeding comes from, it is very important to clarify the source of bleeding. An anal bleeding may conceal a colon carcinoma, which often causes no further discomfort other than bleeding at an early stage. Therefore, every anal bleeding must be clarified!

Therapy:

Slightly enlarged hemorrhoids (1st and 2nd degree) that cause discomfort are usually obliterated. The desolation consists in a completely painless injection of a drug into the enlarged Hämorrhoidalknoten, which then shrink and thus lead to the freedom from symptoms. Depending on the size of the hemorrhoids, multiple injections are required. Another treatment method is infrared treatment or cold therapy. Highly enlarged third degree hemorrhoids are ligated with an elastic rubber ring which is striped over the enlarged hemorrhoids nodule. The thus hardened hemorrhoids are no longer supplied with blood, die off and are excreted after five to ten days with the defecation. Per treatment, only the hardening of a hemorrhoidal knot occurs. If the hemorrhoids are too enlarged (4th degree), however, only one operation will help. So it makes sense to visit a proctologist at the beginning of the procedure and not to endure the problem for years until an operation becomes inevitable. Because a timely initiated therapy can usually avoid surgery.
