To assess fertility, doctors use a spermogram to analyze male seminal fluid according to fixed criteria.
If the desire to father a child does not come true, there are many reasons. These reasons may lie with him or with him, with both or are sometimes not easy to determine.

First of all, important aspects of the man’s fertility can be determined with a spermogram. The doctor analyzes the sperm of the man by a proven standard procedure. The focus is on the microscopic assessment of sperm in terms of their shape, number and mobility.

Standard values of the WHO

The World Health Organization (WHO) set new standard values for a spermiogram in 2010. This new definition is based on the results of ejaculate studies of fertile men. However, if the spermogram is conspicuous, it does not necessarily mean that the man is infertile. For example, stress, smoking or heavy drinking may cause sperm quality to be temporarily impaired. Therefore, the doctor will make a pathological finding with a second sperm examination to confirm a disease. There must be at least 7 days between both examinations.

Why a spermogram is done

The most common reason for a spermiogram is that couples try in vain to get pregnant. With a spermiogram can then check whether a low number or insufficient quality of sperm is responsible for the unfulfilled desire to have children. Also, a spermiogram after sterilization is used to test whether fertile sperm are still present in the ejaculate.

How to get a sperm sample

To have a doctor make a meaningful spermiogram, the sperm must be brought freshly to the examination. The most reliable is a spermiogram when the man masturbates at the site of the spermogram, collecting and dispensing the sample in a sterile sample container. Hospitals and urological practices usually have a quiet space designed specifically for them.

Many men have a problem masturbating in such unfamiliar surroundings. In this case, there is also the opportunity to masturbate at home and then bring the sample as soon as possible in the practice. The doctor will tell you in advance how to store and transport the sample.
For best results, WHO recommends that you abstain sexually for 3-5 days before the rehearsal. So the best possible quality of the sperm can be tested.

Which is examined first with the spermiogram

To create a spermiogram, the sperm is first examined by the doctor or laboratory staff without major aids. The so-called macroscopic examination has the following criteria:

  • Smell: Normal is a chestnut-like odor.
  • Color: Normal whitish, cloudy. A brownish tint may indicate blood. If the hue turns yellowish, it indicates an infection.
  • Volume: According to the WHO, the volume of ejaculate per ejaculation should be more than 1.5 milliliters.
  • PH value: An indicator paper can be used to determine the PH value. It should be between 7.2 and 8. After the macroscopic examination, the microscopic examination follows.
  • Number of spermatozoa: There should be more than 39 million spermatozoa throughout the entire ejaculate.
  • Sperm concentration: The norm here is 15 million sperm per milliliter of ejaculation
  • White blood cells: One milliliter of sperm should contain less than one million white blood cells.
  • Share of Vital Sperm: At least 58% of sperm must be alive. This is usually checked with the help of a dye. Only the dead sperm cells are discolored by this dye.
  • Proportion of motile sperm: At least 40% of sperm must be mobile
  • Proportion of normal-shaped sperm: you should have a regular, normal-shaped head.

How susceptible a spermogram is

The spermiogram may be different from time to time. If the man had an infection, it will show in the spermiogram. The spermogram therefore not only relates to the general health of the man, but also very specifically to the state of the day. This can cause deviations.
How many sperm are needed to conceive??
According to WHO, 15 million healthy sperm are needed. However, very few are enough, in the end only one sperm is fertilized. However, if there are fewer sperm, it can take longer to successfully conceive.

Does the amount of sperm have an effect on fertility?

The amount of sperm has no effect on fertility. Only the number of healthy sperm in the fluid has an effect on the fertility of the man.

How do you improve your sperm quality??

One should eat healthy food in general. A poor diet damages sperm quality. Unfortunately, to improve the sperm, there are no medications. However, a range of foods are recommended.
Here are a few tips to improve sperm quality:

  • A healthy, balanced diet with enough vitamins, zinc and folic acid.
  • Antioxidants can improve sperm quality a lot, as free radicals affect sperm quality a lot
  • Omega-3 fatty acids-> they are contained in walnuts, for example
  • Do without pleasure poisons. That does not mean smoking and moderate drinking.
  • Do not wear too tight underpants and avoid seat heating
  • Pay attention to a regular exercise, but no competitive sport
  • Environmental toxins shun e.g. Bisphenol A (BPA) (plasticizer)

Antibody in the ejaculate

If there are antibodies in the ejaculate, the sperm will be destroyed by these antibodies. It comes to infertility.

Cost of a spermiogram

As a rule, a spermiogram costs between 60 and 90 €. But if you convince the doctor credibly that it does not work with your desire to have children, the health insurance will pay the full cost. But to be sure, you should first contact your health insurance company.

How are you??

Most of the time, a cause for childlessness is found and can therefore be targeted by a doctor. In about 60-70% of the allegedly infertile couples, the desire to have a baby with a medical care but come true. What you two need for it? Good one year patience and stamina! Note: Especially stress affects fertility.
