This article reflects personal experiences made during a wisdom tooth OP when all four wisdom teeth were removed. Of course, it does not replace the consultation with your doctor!

Have you heard from your dentist that you need to have your wisdom teeth removed? That’s for sure not such a nice news, after all, now comes a real operation for you. But nowadays the procedure itself is completely harmless and can also be done under general anesthesia. But why do teeth have to be pulled at all and can not just grow? And how does a wisdom tooth OP actually work? And what do you have to consider before and after? I’ll tell you this and more in the following article.

Why does the wisdom tooth have to go out??

Evolutionary, the wisdom tooth is the third big molar tooth. If it grows in the correct place and in the correct position, it does not have to be removed. Wisdom teeth, however, do not always develop the same, they may lack roots and the number of their cusps varies considerably. Occasionally, they are completely deformed and can not grow properly and are said to be retarded. Retention means that they are basically created, but do not break through. Such a situation can easily cause inflammation and then the wisdom tooth must be removed in any case.

In many cases, another reason is the cause of a wisdom tooth OP. For example, when the wisdom teeth are horizontal and cause problems on the adjacent molar as they grow. And sometimes the wisdom tooth is okay, but the jaw is too tight to take on another tooth. Tension pains and malocclusions would be the result, if the wisdom tooth was not removed. Incidentally, not every person has all four wisdom teeth, some get only three, others get none.

If your dentist has told you that your wisdom teeth have to be removed, then usually one of these problems is present and the teeth should be removed in order to save you follow-up problems. Using an X-ray, the dentist can see how the wisdom teeth are and how far they have developed. For experienced dentists and oral surgeons the wisdom tooth OP is a routine intervention. The costs for the operation are covered by all private and statutory health insurance companies.

Before the operation

Decision between local anesthesia and general anesthesia

Which decision you ultimately make is entirely up to you. A local anesthetic is fundamentally gentler on the body than a general anesthetic and would also have the advantage that you are fit again right after the operation and can go home. However, those who are very afraid of the procedure do not necessarily have to play the brave if they do not feel like it. Then you can also give you a short anesthetic and just sleep through the whole operation.

If you have decided on the local anesthetic, you do not need to prepare yourself specifically. In contrast to surgery under general anesthesia, however, you should never come to the surgery sober. Although the procedure is painless under local anesthesia, it is an exciting affair. So that your cycle remains stable, you should take a good breakfast to you.

If you have decided on the general anesthetic, you must not eat anything on the day of the operation and not drink anything for two hours before the operation. You may be intubated via a breathing tube during surgery, so the stomach must be empty during the procedure.

For a general anesthetic you should ask your health insurance after the reimbursement, some health insurance companies take over the removal of multiple wisdom teeth in an operation, the cost of anesthesia. For me, this did not work, I had to pay for the general anesthesia at that time myself, because my health insurance has taken the cost only for anxious patients. And this I should have certified by a psychologist, which was too much effort for me, as I would have needed several sessions for it. Nevertheless, I opted for general anesthesia and the costs – I mean it was about 240 euros – were borne by myself.

If you have a dental insurance, then you should ask in any case, some providers pay the cost of general anesthesia.

The course of the wisdom tooth OP

If you have received your local anesthetic or are under general anesthetic, removal of the wisdom tooth begins for the dentist or oral surgeon. On the x-ray, he can already see what the tooth is and whether it is likely to be difficult, for example, because the tooth has become wedged. He then opens the mucous membrane over the wisdom tooth and expands the bone of the tooth if necessary, so that the tooth fits through it.

Then he begins with the separation of the tooth. If the tooth is very deformed and wedged, it may be that he first has to break the tooth into smaller pieces. When the tooth is finally removed, the wound is sewn shut. If several wisdom teeth are removed in one operation, the dentist repeats the procedure on all wisdom teeth.

Possible complications of the wisdom tooth OP

As a rule, the surgery runs smoothly, but as in all situations, it can of course come to unpredictable events. If, for example, the wisdom tooth to be removed is very deformed and difficult to grasp, it can slip into the maxillary sinus in whole or in part. Then the dentist has to make a larger surgical opening, which of course leads to a longer suture and a larger wound.

Overall, it can cause swelling in the oral cavity and cheek area. It may also be that you can not open your mouth so far in the first few days. It may also be that you get pain in the operating area after surgery.

To avoid inflammation and pain, your dentist will usually give you a prescription for painkillers and antibiotics right after the operation. The analgesic should be taken before the effect of anesthesia subsides, so that the pain does not arise at all. Since you should spare yourself for the rest of the day, your doctor will issue you a sick leave for the day of surgery, with removal of all four wisdom teeth probably one or two more days longer.

Behavior after the operation

On the day of the operation

Please do not plan any further activities for the day of the operation, but save yourself today. Best place yourself with an increase for the head and cool your cheeks every 20 minutes. Many people have swelling on the cheeks after surgery, which you can dampen with regular cooling.

Caffeine-containing drinks like coffee should definitely be avoided today as caffeine can promote rebleeding. Also, alcohol and nicotine are absolutely forbidden after surgery! Take today maximum Breikost or soups to you, on milk products you should do without the first days completely.

Also on the day of surgery you should clean your teeth, but the surgical wounds while spared. Instead, Chlorhexamed Mouthwash can be used for cleaning. Remember to take the prescribed medication. If you discover that a wound is bleeding or that you are in pain, do not be afraid to visit your dentist again. Even if you get a strong swelling in the face, which goes beyond the expected level, please also seek immediately to your dentist.

In the following days

As a rule, the dentist is on the second day after the operation, a short check at the dentist, if all the sutures are intact and if no inflammations have formed. The sutures themselves remain in your mouth for about ten days and are then pulled.

In the days after the operation you should refrain from sports, heavy lifting and stooping. The wounds have not healed properly and there is a risk of wound opening and inflammation. Even if you can basically work again, you should at least not expose yourself to excessive physical stress. If you still have very strong puffiness on your face, you should continue to cool your cheeks from the outside.

Even if you have the impression that everything is fine, you should take your antibiotics until they are used up. In case of pain, you can continue to take a painkiller. Please continue to pay attention to good oral care. If you have the impression that the seams are no longer bleeding, you can gently clean the areas with a soft toothbrush. Also, the use of Chlorhexamed Mouthwash is still suitable to prevent infection.

If you often have a thread on your tongue in the following days, you need not worry about it. If the wound has healed, the thread is in a very smooth mucous membrane. If the knot loosens, the thread can slip out of the smooth mucous membrane and then lie flat on the tongue. Incidentally, mucous membranes heal very well and very quickly, which you will notice in the days after the operation.

It may take a few days until your face is completely swollen. Even blue cheeks are among the usual side effects, which disappear only a few days later. After a total of 10 days, you can say in principle that everything is done and over. Now you just have to go to your dentist and do a final check. If everything is cured correctly, the dentist pulls the threads within a few minutes, which tweaks a little last time.

At first, it may be frightening to have an operation in front of you. But actually removing the wisdom teeth is a routine intervention that experienced dentists have a good grip on. Surgery is definitely painless and well tolerated by local anesthesia or narcosis. Anyone who has the wisdom tooth OP is usually wondering what he was so afraid of, because the operation itself is not bad.

Even pain can be well counteracted with the timely intake of a painkiller. The days after that, when the face is swollen and blue, and the mouth – especially after removal of all four wisdom teeth – can not be opened properly, are a bit more uncomfortable. But even these unwanted side effects have subsided after a few days, and if then after ten days, the threads are pulled, it’s all over.
